Copayments exclude your monthly premium, and we do not hold premium data, so this compares the cost of the procedure only. Coverage rules, waiting periods and annual maximums still apply. Confirm your own benefits with your carrier before treatment.
Only plans with a published per-procedure copayment schedule can be compared this way. PPO plans generally do not publish their allowed amounts, so they are not listed here.
