When a provider bills for a more expensive service than what was actually provided. Example: billing a Level 5 office visit (99215) when only a routine Level 3 (99213) occurred.
Unbundling
Billing separately for services that should be combined under a single, cheaper code. This artificially inflates your total bill.
Duplicate Charges
The same service appearing multiple times on your bill when it was only performed once. Often a simple billing system error, but you shouldn't pay twice.
Phantom Charges
Charges for services or supplies that were never provided. These often hide behind vague descriptions like 'miscellaneous supplies' or 'administrative fee'.
Balance Billing

How does the bill scanner work?
The scanner reads the text, line items and procedure codes on a provider bill and compares available references and billing patterns. Review each finding against the original document; a flagged code or price difference is a question to verify, not proof of an error.
Is my bill data secure?
Scanning sends your document to our configured AI processing provider after your consent. Extracted details and findings are saved to your account to support later review. See our privacy policy and the upload disclosure for processing and retention details; only upload documents you are authorized to use.
What types of files can I upload?
You can upload PDF bills, images (PNG, JPG, WEBP), or paste the text directly. For best results, ensure your bill shows itemized charges with CPT/HCPCS codes.
What if the scanner misses an error?
Our scanner catches the most common billing errors, but some issues require human review. If your bill seems high but no errors are detected, consider requesting an itemized bill from your provider and using our full negotiation tools.
Can I dispute the errors found?
Absolutely! For each error detected, we provide a dispute letter snippet you can use. You can also use our Bill Negotiation Hub to generate a complete, professional dispute letter.
Does this work for EOBs (Explanation of Benefits)?
EOBs need an insurance review, not bill-price negotiation. Use Bill Audit to scan an EOB and transfer its stated amounts to the EOB review form, or enter its details directly in the EOB Analyzer. An EOB is not a bill or proof of an amount the provider may collect.
